Quickly and easily schedule with Pet Well Mobile Vet an appointment time that is convenient for you and your pet.

This is not an automatic process. Once we receive your request and all required documentation, we will contact you to confirm a date and time that is as close to your request as possible. If you are a new or inactive (no appointments for 18 months) client, please see the required documentation below the form.

This form should only be used for requesting appointments that will take place at least 2 full business days after the time of the submission of this form.

We are genuinely delighted you chose Pet Well Mobile Vet to be a part of your pet's wellness journey. To help everything go smoothly and ensure the best experience for all our beloved clients and staff, here are a few friendly reminders about how we operate:

Veterinarian-Client-Patient Relationship (VCPR)
In Michigan, a Veterinarian-Client-Patient Relationship (VCPR) must be established through a recent in-person examination of the pet, and the Veterinarian must assume responsibility for the pet's care. A pet owner's responsibilities for a VCPR includes agreeing to follow the Veterinarian's instructions, providing necessary information about the pet's history, and ensuring the Veterinarian has sufficient knowledge of the pet.

Required Documents
We need a complete picture of your pet’s health to provide the best care! We kindly ask that you:

  • Send us previous records: Please contact all previous vet clinics and ask them to email your pet's full medical history to PetWellMobileVet@gmail.com before your appointment is confirmed.
  • Let us know if they're new: If your pet has never visited a vet before, just let us know.
  • Secure your spot: A non-refundable deposit is needed to confirm appointments for new or inactive clients, and for all surgeries. Please keep an eye on your email for a secure payment link, which is active for 48 hours.

Rabies Vaccinations
For the safety of all pets, our staff, and the community, a current Rabies Vaccination is a legal requirement. We ask that all pets in our care maintain current vaccination status. If your pet's vaccination is not current or we do not have a record on file, we will administer a Rabies Vaccine during the appointment, provided they are healthy enough to receive one at that time.

Payment Information
Payment is due in full at the time of service. We accept cash, checks, and major credit/debit cards, including Discover, Mastercard, and Visa (we apologize, but we cannot accept American Express or Care Credit at this time).

We value your time and appreciate notice if you are unable to keep your appointment. Please call or text us at least 24 hours prior to your scheduled appointment or surgery time to avoid a $75 no-show fee.

We send a big thank you to all our clients who make timely payments! For any outstanding balances, a late fee will be applied to delinquent accounts after each 30-day period. Accounts remaining overdue after 60 days may unfortunately result in legal action.

Zero Tolerance Policy
We pride ourselves on building a positive, professional, and respectful relationship with you and your cherished pets. As we provide care in the comfort of your home, maintaining a safe and secure environment is paramount. We have a Zero-Tolerance Policy for any behavior that makes our staff feel unsafe or disrespected. This includes, but is not limited to, profanity, yelling, demanding behavior, disparaging comments, rudeness, threats, acts of violence, political comments, racial slurs, or any form of harassment. Such behavior will result in the immediate termination of our professional relationship.

For the health and safety of our staff, who may have allergies or sensitivities, please refrain from smoking or vaping during the duration of your appointment. Thank you for your consideration.
