Please use the form below to request an appointment with Pet Well Mobile Vet!

Quickly and easily schedule an appointment time that is convenient for you and your pet.

This is not an automatic process. Once we receive your request and all required documentation, we will contact you to confirm a date and time that is as close to your request as possible. If you are a new or former client, please see the required documentation below the form.

This form should only be used for requesting appointments that will take place at least 2 full business days after the time of the submission of this form.

*Complete medical records from all previous veterinary clinics where your pet has received care are required prior to your request being confirmed. Please call the clinic(s) and have them email the records to PetWellMobileVet@gmail.com. If your pet has never been seen by a Veterinarian before, please indicate that.
*New or former clients and all Surgery Appointments require a non-refundable deposit. We will contact you by email with a secure payment link. This link is active for 48 hours.
All pets in my care must maintain a current Rabies vaccine. This is not only for their health and yours, its the law. If they are not current or I do not have a current record, a Rabies vaccine will be administered if they are healthy enough to receive one at the time of the appointment.
Payment is due in full at time of service. Payment can be made by cash, check or credit/debit card (Discover, Mastercard and Visa, Sorry-no American Express). Failure to call/text 24 hours prior to a scheduled appointment or surgery time will result in a $75 No Show Fee. A Late Fee will be applied to all delinquent accounts after each 30 days a balance remains on the account. Legal action will be taken against any delinquent accounts after 60 days. Finally, Pet Well Mobile Vet has a Zero Tolerance Policy. The following behaviors will result in termination of our relationship with you permanently: profanity, yelling, screaming, demanding, entitled behavior, disparaging comments in person or online, rudeness, threats, acts of violence, political comments, racial slurs, or any behavior that makes my staff or I feel unsafe. We are in your home and develop an intimate relationship with you and your pets. We need it to be safe for everyone.
